Dry Eyes, Computer Vision Syndrome & Eyelid Hygiene

Dry Eyes, Computer Vision Syndrome & Eyelid Hygiene

Tears are formed by tiny glands that surround the eye. The tear film is comprised of three layers: a top-most oil layer, a middle water layer, and lower mucous layer (closest to the eye). The tear film lipid layer is the major barrier to evaporation of tears from the ocular surface. A decrease in its thickness or functional integrity may cause evaporative dry eyes. When the meibomian glands get blocked or do not function properly, the tear film is more likely to evaporate. In today’s world, with so much pollution and the rampant use of eye make up, these oil glands tend to get blocked easily & may be the major cause of dry eyes world over. The eye mascara (kaajal), eye shadow and eyeliner can all contribute to the dry eyes, by blocking the meibomian glands. The eyelids, when they blink, assist in the distribution and the replenishing of the tear film. So, when you look at the computer screen for a long period of time, the eyelids do not blink normally & the tear film is not replenished, leading to dry eyes (computer vision syndrome). This can contribute to dry, itchy, gritty and uncomfortable eyes. Thus, the eyelids play a very important role in maintaining the tear film & preventing dry eyes, by production of the tear lipid (oil) layer & by blinking. Dry Eyes can be prevented / treated with adequate lid hygiene (cleaning, applying dry heat & massaging), & relaxation of the eyes (by staring into the distance every 10 – 15 minutes & blinking regularly). Removal of eye make up adequately every night helps to prevent & control the dry eye syndrome. Continue reading

Congenital Ptosis (Drooping Eyelids in Children) Surgery Treatment

Congenital Ptosis (Drooping Eyelids in Children) Surgery Treatment

Ptosis (Droopy Eyes / Droopy Eyelids) can affect one eye or both eyes. Ptosis may be present at birth, or may be acquired later in life. If a droopy eyelid/ droopy eyes are present at birth or within the first year of life, the condition is called congenital ptosis. In most cases of congenital ptosis, the cause is idiopathic (unknown). Congenital blepharoptosis has physical, functional, and psychological consequences. . Although the primary reason for the repair is functional, the oculoplastic surgeon has an opportunity through this procedure to produce symmetry in lid height, contour, and eyelid crease for better cosmesis. A good eyelid surgeon understands that ptosis correction has to have a cosmetic (aesthetic) thought process – When a child looks at the world, the world looks at their eyes – And, so, the eyes must look the same (symmetrical eyelid heights) and beautiful, post surgery. Dr. Debraj Shome discusses on ptosis in children, aetiology, causes, complications & the types of ptosis surgery for the treatment (cure) of the same. Continue reading

Proptosis, Exophthalmos, Orbital Disease & Orbital Surgery

Proptosis, Exophthalmos, Orbital Disease & Orbital Surgery

The orbit is the bony socket surrounding the eye. Any disease of the orbit (orbital disorder) can cause displacement, proptosis and exophthalmos of the eyeball and lead to impairment of vision. The commonest disease affecting the orbit is Grave’s disease (Thyroid Eye Disease). Tumors (Neoplasms) can also affect the orbit, commonest being orbital lymphoma, lacrimal tumors like pleomorphic adenoma and vascular tumors like cavernous hemangioma and lymphangioma. Treatment of orbital tumors (neoplasms) and other orbital lesions is by using a type of orbital surgery called an orbitotomy. Thyroid Eye Disease is treated with an orbital decompression surgery. Orbital surgeries are extremely specialized surgeries performed by oculoplastic surgeons. Watery Eyes Treatment: Blocked Tear Duct Surgery

Watery Eyes Treatment: Blocked Tear Duct Surgery

Lacrimal Duct Obstruction / Blocked Lacrimal Duct / Lacrimal Duct Stenosis causes Watering (runny / tearing) Eyes. Blocked Tear Ducts (NasoLacrimal Duct Obstruction) can occur in adults, children, babies, infants and newborns and result in watery / tearing / runny eyes (Blocked tear duct symptoms). Watering eyes interfere with vision. In addition, due to the nasolacrimal duct obstruction, eyes can get repeatedly infected, causing dacryocystitis (lacrimal sac infection) and endophthalmities (eye infection). Treatment consists of making an artificial opening on the medial side of the eye into the nose to bypass the blocked nasolacrimal duct and re-establish the normal flow of tears into the nose. This surgery is called Dacryocystorhinostomy (DCR Surgery) and can be performed through a skin incision (External DCR), through an endoscope inserted through the nose (Endonasal endoscopic DCR) or via laser (LASER assisted DCR). Surgery is very successful and can easily treat this problem, which if not treated as soon as possible, may cause complete blindness due to severe eye infections. Dr. Debraj Shome, one of the top / best oculoplastic surgeons in the world, blogs on the symptoms and treatment of this common problem. Continue reading

Lip Augmentation via Lip Injections: Are Bigger Lips More Feminine?

Lip Augmentation via Lip Injections: Are Bigger Lips More Feminine?

Full lips with an accentuated border have often been associated with beauty and youth. In studies performed on the science of human attraction, psychologists have concluded that a woman’s facial and sexual attractiveness are closely linked to the makeup of her hormones during puberty and development. It has been shown that the more oestrogen a woman has, the larger her eyes and the fuller her lips, characteristics which are supposedly perceived as more feminine. Surveys performed by sexual psychologists have also found that universally, men find a woman’s full lips to be more sexually attractive than lips that are less so. It is very easily possible nowadays to give a lady more attractive, curvaceous, fuller, lips, using cosmetic filler lip injections with Fillers like Restylane and Juvederm. Restylane / Juvederm are very safe, affordable, predictable and injections with Restylane / Juvederm Fillers can be performed in the doctor’s office itself. Dr. Debraj Shome explores the advent of lip filler injections and explores the data on whether fuller lips are indeed more feminine. Continue reading

Liposuction or Face Exercises to Make a Round / Square Face Shape Oval?!

With respect to beauty, the face shape plays a part. Though every person’s face is unique, it’s generally accepted that there are seven basic face shapes. The ideal face shape is oval. What if you do not have an oval face and have a square or a round, broad looking face? The most important step is to evaluate the reason for the square or round face shape. Is it the fat below the facial skin? Is it the muscle mass of the mass (i.e. masseter hypertrophy)? Or, is it the bone structure itself of the face? A careful examination by your plastic surgeon can help find the cause and treat the same. The face shape can then be altered by VASER liposuction if fat is responsible or if masseter hypertrophy (face muscle enlargement) is responsible, Botulinum toxin injections (BOTOX treatment) can be used to treat masseter hypertrophy. Artificial Prosthetic (False) Eye Fitting Post Enucleation Evisceration Eye Removal Surgery

Pthisis bulbi, also called Atrophic bulbi / Small Eye / Shrunken Eye, occurs post eye (ocular) trauma (injury), eye surgery and eye infections (inflammations). Phthisis Bulbi or a painful, Blind eye does not have any vision (sight). Moreover, the blind eye can cause sympathetic ophthalmitis (a disease), which can in turn cause blindness even in the other normal eye. Hence, in an eye with no visual potential, it may make sense to remove the eye (via enucleation, evisceration, eye removal surgery), insert a silicone / acrylic ball implant surgically (to replace the volume of the removed eye) and instead fit in an artificial, custom made ocular prosthesis (false eye). A well fitted ocular prosthesis looks and moves just like a normal eye and it can be difficult even for people to tell if the eye is false or real. Dr. Debraj Shome is one of the top/ best oculoplastic & facial plastic surgeons in the world and currently practices in Mumbai, India. In this blogpost, Dr. Debraj Shome writes about eye removal surgery (enucleation, evisceration, exenteration), a blind eye and cosmetic treatment and rehabilitation of small, shrunken eyes with surgery followed by fitting an artificial, custom, hand made, prosthetic eye. VASER Facial Liposuction – Getting The Correct Jaw Contour & Getting Rid of the Double Chin

The New Technique for altering facial shapes and removing double chins – VASER Liposuction. VASER stands for Vibration Amplification of Sound Energy at Resonance. Simply put, it adds the benefit of ultrasound technology to the standard Liposuction Procedure. Ultrasound waves are sound waves at specific frequency and one commonly comes across it during ultrasound cleaning of dental plaque and while looking at the growing baby in mother’s womb. Vaser has resurrected ultrasound in liposuction. The speciality of Vaser is “ liposelection” – meaning it caused damage to fat cells only without damaging other tissues. Simply put, the VASER technology converts by sound energy the thick fat into almost liquid milk-shake like consistency. As this new type of fat removal causes much less ‘collateral’ tissue damage, VASER is used more and more for delicate fat removal from the face. The face, as we know, is a myriad of many nerves and vessels and any damage to a nerve or vessel has permanent repercussions on the way the face animates and looks. This is where VASER is specifically handy as the VASER energy can just melt away stubborn fat from delicate areas like the jaw line, cheeks, chin and convert a generally ‘round’ face to an ‘angular’ or more pleasant ‘oval’ one. Dr. Debraj Shome blogs on the use of this exciting new technology in facial liposculpture and removal of double chins, chubby cheeks and changing facial shapes. Continue reading
